Missoula, MT—Nine Western Montana Providence Medical Group medical clinics have received national recognition as Level 3 Patient-Centered Medical Homes, the highest accreditation possible, by the National Committee of Quality Assurance (NCQA).
Level 3 status involves meeting and documenting requisite elements in six categories, such as tracking and coordinating patient care, easing access to providers through improved hours and scheduling, and measuring and improving clinic/provider performance. NCQA recognition ensures that these clinics use evidence-based, patient-centered processes that focus on highly-coordinated care and long‐term interactive relationships between the care teams and the patient.

A patient-centered medical home is a model of primary care that combines teamwork and information technology to improve care, improve the patient’s care experience, and reduce costs. Medical homes foster ongoing partnerships between patients and their clinicians using multi-disciplinary care teams to coordinate treatment, follow-up and support. Research confirms that medical homes advance the “triple aim” of higher quality, better patient experience and cost containment.
