As a Patient Centered Medical Home, Grant Creek Family Medicine provides increased access to healthcare for patients – including preventive healthcare. Contact your insurance carrier to find out which preventive screenings are covered under your plan.

Our goal is to provide you with the same care we’d expect for our own family. We specialize in primary care for patients of all ages. We want to build a lasting relationship with you and be your medical home for all your health care needs.

Patients can expect evidence-based care from their physician and team, as well as support for self-management of their health and health care.

In partnership with PAML laboratory services, Grant Creek Family Medicine is equipped with a full spectrum laboratory for onsite testing from 7:30 a.m.-1:20 p.m.

Our Services

  • Primary Care

    Your primary health care provider diagnoses and treats both acute and chronic illnesses. He or she can also help you stay healthy and protect you from illness, provide counseling teach you about your condition and manage your care. Your primary care doctor's practice may include various types of health professionals. Primary care doctors fall into two categories: family medicine and internal medicine. The biggest difference is internal medicine doctors focus on the prevention, diagnosis and treatment of adult diseases, where family medicine doctors focus on meeting the needs of the entire family and care for the whole person throughout his or her life - from birth to old age.
  • Pediatrics

    Providence pediatric doctors, medical providers and caring staff provide pediatric care including preventative checkups, sports physicals, development tracking and diagnostics. We provide a kid-friendly environment that puts children at ease and keeps them healthy through all stages of infancy, childhood and early adolescence.
  • Urgent Care

    Urgent care is a cost-effective alternative to the emergency department for illnesses or injuries that aren’t life threatening. Call 911 or go to the nearest emergency department if someone is experiencing bleeding that won’t stop, shortness of breath or unconsciousness.
  • Blood Work and Medical Laboratory Testing

    Lab work, such as blood tests, help diagnose, treat or monitor your health and provide information that cannot be found on a physical exam.
  • Mammography

    For women who aren’t having any breast cancer symptoms, we offer a screening mammogram, an X-ray test of the breasts (mammary glands) used to screen for breast problems, such as a lump, and whether a lump is fluid-filled (a cyst) or a solid mass.
  • Medication Assistance

    Medication assistance coordinators are available to find resources to help patients pay for their medicine.
  • Well-Baby and Well-Child Checks

    It’s important to understand your child’s growth and development – physical changes such as height and weight, developmental milestones like speech, as well as emotional and behavioral changes. Your child’s well-child check is the perfect time to discuss your concerns and ask questions of your primary care physician.
