Medical Records

Providence Health & Services provides access to medical records from our Hospital to our patients and their authorized representatives.

Receive a copy of your medical record:

To receive a copy of your medical record, please complete the Authorization to use, Disclose & Release Protected Health Information form and fax, mail or deliver it in person to our Health Information Management department. Your request will be processed approximately seven to ten business days from receipt of the completed form. If there is a charge associated with your request, an invoice will be mailed to you and payment for copying records will be due prior to receiving your records. If picking your records up in person you must present a picture ID.

Review a copy of your record:

To view a copy of your record, please complete the Authorization to Use, Disclose & Release Protected Health Information form and call 406-329-5737 to set up an appointment so that you may view your records in the Health Information Department.

How to Obtain a Birth Certificate

If you were born in Montana, most Montana birth certificates may be obtained at the Clerk and Recorder's office in your local court house, even if you were born in another Montana county.
