Medical Records

Providence Health & Services provides access to medical records from our Hospital to our patients and their authorized representatives.

To receive a copy of your medical record:

Complete the Patient Request to Access a Designated Record Set form and fax, mail or deliver it in person to our Health Information Management department. Your request will be processed approximately seven to ten business days from receipt of the completed form. If picking your records up in person you must present a picture ID.

To disclose a copy of your medical record to someone other than yourself:

Complete the Authorization to Use, Disclose & Release Protected Health Information form and fax, mail or deliver it in person to our Health Information Management department. Your request will be processed approximately seven to ten business days from receipt of the completed form.

To review a copy of your record:

Complete the Patient Request to Access a Designated Record Set and call (406) 883-8427 to set up an appointment to view your Hospital records or (406) 883-5680 to set up an appointment to view your Clinic records.

How to Obtain a Birth Certificate:

If you were born in Montana, most Montana birth certificates may be obtained at the Clerk and Recorder's office in your local court house, even if you were born in another Montana county.
