Preventive Health Care

As a Patient Centered Medical Home, Grant Creek Family Medicine provides increased access to healthcare for patients – including preventive healthcare. Contact your insurance carrier to find out which preventive screenings are covered under your plan. It’s possible you don’t have to pay a co-pay or apply expenses to deductible for the following services:

  • Blood pressure, diabetes, and cholesterol tests
  • Counseling to help you:
  • Stop smoking
  • Reduce alcohol consumption
  • Lose weight
  • Eat healthier
  • Manage chronic disease, including diabetes
  • Counseling, screening and vaccines to ensure healthy pregnancies
  • Flu and pneumonia shots
  • Cancer screenings that include mammograms, prostate specific antigen (PSA) tests, cervical screenings and colonoscopies
  • Physicals
  • Standard immunizations and vaccinations
  • Routine well-baby and well-child checkups, from infancy through adolescence

What to Bring

  • Photo ID and insurance card
  • Referral from your primary care physician (if required by your insurance plan)
  • Co-payment (if required by your insurance plan)
  • Full list of medications with dosages (including over-the-counter supplements)
  • Detailed history of symptoms and treatments
  • List of questions

Please arrive 15 minutes before your scheduled appointment so you have time to check in and fill out any necessary paperwork.
