Patient health information – such as demographics, medical history, medications, treatment records and test results – will be available online so that care providers will have access to the information they need at the time and place they need it. This level of access enables highly comprehensive and coordinated care for patients. Patients will also be able to access their electronic health record through the MyChart patient portal, allowing them broader participation in their care.
Participating hospitals and medical practices are able to maintain their financial autonomy by having separate financial and patient schedules, while patient-level data is integrated to create one comprehensive, health record for each patient.
